1. Please re-read Item 1 on the Issues Report dated 5/16/2007; this issue reported on two previous reviews still has not been addressed in this current resubmittal.

2. AS PER ORDINANCE # 98-42 “An additional fee due and payable at permitting shall be $100 for any second resubmission of plans. Per FL. Statute 553.80(2)(b), on any third and subsequent resubmissions that do not correct a code violation that is specifically and continuously noted in each previous rejection, a fee of four times the amount of the fee attributed to plan review will be charged. AN ADDITIONAL FEE OF $100.00 WILL BE ASSESSED UPON PERMITTING FOR THIS APPLICATION.
