Plan REJECTED for 2104 sqft interior renovation of existing nursing home. The renovation includes phase 1 admin renovation, phase 2 nurse stations, phase 3 common bathrooms, phase 4 corridor floor finish. This is reviewed under 2007 Fl Fire Prevention Code as health care and FAC 69A-53.

It is unclear who the original engineer of record (EOR) was for this fire sprinkler system or fire alarm, who set forth the sprinkler design criteria and fire alarm criteria. However the interior renovation plans were submitted with a seal from James Williams for fire sprinkler and Jason Hochstein for fire alarm.
Per FAC 61G15-32.002(5)(6) after the Fire protection system engineering documents (Criteria) is submitted with the original building plans, if work on the sprinkler system does not affect more than 49 heads or if work on the fire alarm is not more than $5000, then no additional engineering input or seal is required.
Please either: 1) Resubmit sprinkler and fire alarm pages that are not signed and sealed. or 2) Have James Williams and Jason Hochstein submit a registered letter to original engineers seeking to reuse his already sealed contract documents therefore assuming all legal responsibility for the system in accordance with FAC 61G15-27.001. or 3) Have the owner or project engineer submit a letter to our office completely removing the orginal engineer as engineer of record for the project or 4)Have the original engineer provide documentation to the delegated engineers and the EOR shall review design documents.

It appears the main entrance will be affected with this interior renovation. Please provide a plan showing or explaining how Fire Rescue is to access the building in case of emergency.

Please acknowledge if the fire sprinkler or fire alarm systems are out of service more than 4 hours that Fire Rescue shall be notified at 904-829-2228 and that a Fire Watch will be conducted.
