Plan REJECTED for interior build out for what appears to be suites 204 to be interconnected to 205 as well as suites 208 and 209 which are not interconnected. The square footage shown on the Clearance sheet of 1804 sqft does not match the total square footage of the 4 suites combined, 2033 sqft. Please contact Lisa Brown at 209-0692, Application Review Supervisor to correct your clearance sheet.

Suite 204 was part of suites 201-204 known as Riverside Pain Physicians. Is 204 and 205 now also part of that business or stand alone? If 204 and 205 will be part of Riverside Pain Physicians, please provide an overall floor plan to ensure all life safety requirements are met such as exits, travel distance, etc.

The cover sheet should reflect the entire project. Currently the cover sheet only covers suites 208 and 209. It looks like a page for suites 204 and 205 was added at the last minute. Also regarding the cover sheet, it should reflect that the Florida Fire Prevention Code 5th edition adopted the 2010 edition of NFPA 101 Life Safety Code, not 2014. Please update your cover sheet.

Pg A1.3 appears incomplete. No outside walls or doors are shown for suites 204 and 205. If suites 204 and 205 are part of this project please provide the same type of details that were provided for suites 208 and 209.

General notes: Your re-submittal shall include two complete sets of plans. Please insert the revised pages into the plan sets, and remove the old pages that are no longer applicable and RESTAPLE the Job Copy of the plan. Resubmit those old JOB COPY pages marked as "old set" and submit with the revised plan set.
If old pages are not resubmitted there may be an additional plan review fee charged due to the amount of time necessary to review entire drawing again.
ALL ITEMS REVISED ON PLANS AND DOCUMENTS SHALL BE IDENTIFIED by circling, clouding or other legible method. ADDITIONAL ITEMS MAY BE REQUIRED - DEPENDING ON INFORMATION AND CLARITY OF FINAL PLANS SUBMITTED FOR REVIEW.
