1.  Med Gas testing:  Please provide a proposal from a qualified medical gas inspection and testing agency. The agency is to perform tests and inspections of the medical gas piping.

The proposal is to include: 

Test and Inspections
1 Perform test, inspection, verification and certification of medical compressed oxygen piping systems concurrently with tests, inspections, and certification of medical vacuum piping and medical gas piping systems. 
2 Perform the following installer tests according to requirements in NFPA 99 and ASSE Standard #6010 Initial blow down, initial pressure test, cross- connection test, piping purging test, standing pressure test for positive-pressure medical compressed oxygen piping, and repair leaks and retest until no leaks exist.

System Verification
3 Comply with requirements in NFPA 99, ASSE Standard #6020 and ASSE Standard #6030 for verification of medical compressed oxygen piping systems and perform the following tests and inspections. Standard pressure test, individual-pressuration or pressure-differential, cross-connection test, valve test, master and area alarm tests, piping purge test, piping particulate test, piping purity test, final tie-in test, operational pressure test, medical purity test, and verify correct labeling of equipment and components.

Testing Certification
4 Certify that specified tests, inspections, and procedures have been performed and certify results. Inspections performed, procedures, materials, and gases used, test methods used, and results of tests.

2.  Beam Connection: Sheet S2.11 shows an existing beam end connected to a new 14" LVL using a Simpson PS 418 strap. Simpson designed this strap at a tension member. Please provide calculation to show that the old beam will be held rigidly and not slide in the direction of the floor.

3. Code Analysis:  Please revise the wind speed and exposure shown on G1.11 code analysis to comply with the 2010 Florida Building Code.

4. Product Approval: Please provide product approval for the exterior windows per State Statue 553.842 and Florida Administrative Code 9B-72. 842 and Florida Administrative Code 9B-72. Please use the St Johns County Product Approval Specification Sheet.

5. HVAC Equipment Schedule: Please provide an HVAC equipment schedule showing total cooling in tons, auxiliary heat, supply air, and outside air.

6. Drive-up Canopy: The plans show the removal of the drive-up canopy. Please provide details for the removal and repair of the south elevation.

7.  Trap Primers: Please provide trap primers for the floor drains.

8. Medical Vacuum Pump: Please revise the plumbing riser at the vacuum pump to comply with FBC-P 713.6 and 713.7.

9. Electrical Riser Diagram/Fault Current:  Sheet D1.11 shows the existing power panels removed and relocated. If so please provide a fault current letter and an electrical riser diagram that includes the meter, disconnect, conduit size, wire size (AL or CU), grounding, panel, ampacity of the service, voltage, and phase.
